Lucy died on 23 September after going missing from a psychiatric unit in Scarborough where she had been staying as a voluntary inpatient.

Lucy was admitted to a psychiatric unit on a voluntary basis, which meant she could come and go freely.

“I was always in trouble for something, and they were always adjusting the meds or sticking me in the psychiatric unit for something.”

Due to this she became "very depressed and suicidal and was admitted to psychiatric unit for six months of her life".

The country's only psychiatric unit was last year criticised by the ombudsman - an official whose job it is to look after the public's interests - for not having had a psychiatrist since 2017.
